LIABILITY WAIVER & MEDICAL RELEASE Soccer and the goalkeeping position are physical activities that have certain inherent risks that could result in physical harm, and, in extreme cases, death. I understand the risks involved and I hereby release the Southwest Goalkeeping Academy and it's employees from any and all liability for any physical injuries or illness that may occur to the above named athlete while attending training. I have no knowledge of any physical impairment that would be affected by the above named athlete's participation in training activities. I also verify that the above named athlete is covered under the insurance policy listed in this form. Furthermore, I hereby authorize the staff of Southwest Goalkeeping Academy to act on my behalf according to their best judgment in any emergency situation requiring medical attention. *

  • Payments are made monthly, in advance, and are due at the first session of the month

  • We accept cash, check or credit card payments

  • No refunds for missed sessions

  • Missed sessions may be only be made up during the same month

  • Availability of make-up sessions is not guaranteed

  • Suspended or dismissed athletes will not receive refunds

  • By registering to our program, you agree to the Academy Code Of Conduct




  • A completed registration is required prior to training participation

  • Tuition payments are made for one month, in advance

  • Payments are due at the first training session of each month


  • Parents are always welcome to attend and observe training sessions; this is the best way to learn how the program works

  • Parents must not instruct athletes during training


Each athlete is expected to conduct themselves in a sportsmanlike manner. Good sportsmanship includes, but is not limited to:

  • Respect for all coaches, staff, volunteers, fellow training mates, and competitors

  • Respect for the training facilities, and for all private and public property

  • Display of courtesy and good manners during training and games

  • Self-control, responsible behavior, consideration of others’ physical and emotional well-being; no profane or abusive language or behavior

  • A positive attitude

  • Honesty—no cheating or lying

Athletes are expected to: 

  • Attend all training activities of their program

  • Be on time, and arrive ready to train

  • Put forth their best effort

Soccer, and especially goalkeeping, is a potentially dangerous activity. We will be teaching safety skills and respect for other players. Athletes who do not cooperate with our guidelines, or exhibit dangerous or uncooperative behavior, will be suspended or dismissed. Excessive absence from training, disruptive or inappropriate behavior during training may result in suspension or dismissal. Athletes that are under the influence of alcohol or drugs are suspended immediately.
